My Treatment Approach to Azoospermia

The treatment begins with a thorough evaluation of all reports. The primary objective of this assessment is to ascertain the specific level of stem cells, as different treatment approaches are applied based on this level.

 We employ two main treatment methods that complement each other (a drug-based approach with positive mutual effects). This involves administering gonadotropin hormones and phytotherapeutic nutritional supplements tailored to the specific stage of development and examination.

We adopt two distinct approaches for monitoring. The first entails tracking hormonal criteria through blood tests, which is crucial for precision. In order to enhance the precision of information and consequently determine the appropriate treatment, it is crucial to incorporate both the approaches. Employing both methods is vital to ensure accuracy, minimizing the risk of incomplete data that may result in misinterpretation. For international patients, we offer the option of examining semen using a biologically compatible carrier material. If authentic sperm cells are detected more than once in this carrier material, it would be advisable to schedule an in-person spermiogram examination simultaneously.

As an illustration, in cases where no stem cells are present at a particular stage, we exclusively administer hCG, the initial gonadotropin. This hormone is utilized to stimulate the production of additional stem cells within the base membrane, and we provide complementary support using phytotherapy tailored for this specific stage. Following a similar rationale, we progress to the second and third gonadotropins for treatment during the final stage of maturation.

 The treatment duration may extend for a minimum of two years, particularly for patients in the earliest stage. Our monitoring and treatment protocols are structured in three-month intervals. The duration of treatment can vary based on how well the patient responds to the treatment. As each patient's case is unique, offering a precise average timeline is challenging.

It's important to note that not every patient may have a suitable micro-environment for sperm development, leading to cases where mature sperm may not form. Our goal is to at least reach the level of sd4 elongated spermatid. If mature sperm cannot be found in regular semen ejaculate samples, we first explore MESA with epididymis aspiration. If this yields no results, and sperm can be obtained through micro TESE surgery, we proceed with classical ICSI using spermatids, and if elongated spermatids are available, we opt for in vitro fertilization with ELSI. In cases where spermatids are present, we perform IVF with ROSI, and we utilize piezo electrical stimulation to enhance the chances of success.

Our objective is to develop Day 5 embryos. While we wait for the genetic laboratory results, we implement a new generation genetic screening (NGS) to choose healthy embryos. This involves obtaining a biopsy from the outer layer while allowing the overstimulated egg and the uterus to rest. Once a healthy embryo is identified, we aim to transfer it to the prospective mother's uterus in the subsequent month at the earliest convenience. To assess pregnancy progress, we closely monitor beta hCG levels through blood tests, typically conducted around 10 days following the embryo transfer.
