From Fertility Struggles to Medical Triumph: A Journey of Hope and Success by Dr. Celalettin Peru

At the age of 26, I took the leap into married life. I had spent the last two years working diligently as an internist assistant, paving the way for what seemed like a promising future. However, life had its own plans, and I found myself facing an unexpected challenge.

It all began when I decided to undergo a routine check-up, including a sperm count test. The results revealed that I had a sperm count of about 25-30 million, but the sperm were weak in both movement and morphology. The doctor advised me to consider a microscopic varicocelectomy, suggesting that the culprit might be a varicocele in my left testicle. Worried about my slightly elevated FSH values, I agreed to undergo a biopsy of my left testicle during the operation.

The biopsy results were disheartening, indicating a mild to moderate spermatogenesis disorder. My journey had taken an unexpected turn. After the surgery, complications arose. My left testicle swelled and caused excruciating pain. An infection set in, leading to orchitis and epididymitis. As a result, my sperm count plummeted to a mere 1.5 million.

My partner and I were determined to start a family, so we tried giving it time and attempted artificial insemination, but success eluded us. In the end, we welcomed our first child into the world through in vitro fertilization (IVF). Six long years passed before we decided to try again, but the second IVF attempt proved unsuccessful. It was not until the third attempt that we finally held our second bundle of joy.

Through all the ups and downs, I learned a great deal. People often asked me how I managed to navigate this challenging journey. In a role of amateur guidance and advice, I began sharing my experiences and knowledge with dozens of individuals facing similar struggles. With the guidance of my esteemed professor, Refik Tanakol, I started assisting in the treatment of cases of hypophyseal hypogonadism. Later, I embarked on my own journey, assisting in the treatment of testicular cases of azoospermia.

The cases of "hypo-hypo" as I liked to call them, yielded significant successes. Among the azoospermic cases treated through this approach, five out of five had children, causing a stir among patients. While I had focused on "hypo-hypo" cases between 2005 and 2012, the number of azoospermic cases grew significantly after 2012.

My journey was marked by the triumph of my patients over seemingly insurmountable odds. From those with Sertoli cell-only syndrome, considered irreversible, to individuals who had a testicle removed due to a tumor, to cases of congenital testicular agenesis and advanced atrophy, and even Klinefelter syndrome and various genetic Y deletions, we experienced a wealth of successes.

Now, as I reflect on this remarkable journey, I promise to leave a meaningful legacy and offer guidance to both patients and physicians. With a heart full of gratitude and the hope that this work may be of help to others, I share these experiences and insights, with faith in the good deeds that will live on beyond my time.
